Quad Pole, Double Throw (4PDT), latching footswitch with 12 solder lugs.

Carling SPDT latching footswitch used in footswitch boxes and effects pedals (112-P). This style of On-On footswitch can be found in many vintage stomp boxes and channel/effect footswitches. The two identical lugs are the two throws and the lug with a unique shape is the common pole.
